Keep in mind that these details are based on a brief encounter with the tape and memories of it a few years later...
Back in the summer of '93 Jeff Braine borrowed a few promo cassettes from his friend's sister, who worked at Geffen UK. One of the tapes was In Utero. He describes it as:
-
"...it was just a laser-printed cardboard/thick paper (can't quite remember now) insert. Geffen logo on the front (monochrome)."
When I asked Jeff to compare an Incesticide promo cassette's insert to the promo he borrowed, he said it was very similar, but:

"...the one I had was black ink..."
This almost describes another surfaced In Utero promo perfectly, and it too has an extra song(s):

"...had all the album songs on it plus 'Gallons,' 'MV,' 'VCV,' 'Marigold' and 'IHMAWTD.' "
That's surprising, but Jeff noted:

"...however the promo tape *did* suffer from drop-out and sounded a bit more 'raw' (could have been just me, but..)...I'd guess they just used cheaper tape or something for the promo releases - OR the version I got to borrow was the one before the slight remix (where they just added a bit of volume to vocals and did a bit of EQ tweaking)."
Jeff also asked about a promo cassette for Nevermind.
