The artwork on the front of the sleeve is just like the official releases.  The back is quite different, though.  "Nirvana" is printed with a font similar to the 'official' Nirvana font, but it's not quite right.  The font used for "Nevermind" is unlike anything I can recall on any DGC Nirvana release.  The watery artwork is absent and instead of the lovable Chim Chim, one of the blurry Michael Lavine photos is used with the track list below it.  Songs are shown out of order on the sleeve, as well as on the vinyl labels.

The top right corner of the back of the sleeve is not shown, but it's the only external location that ART-1004 appears.  The spine only has the band name and album title.  The omission of a record label that produced this can't be a good sign, right?  For what it's worth, the sleeve is constructed with "thick and hard paper" according to one owner.

Labels on the black vinyl are red with black text.  Both are rather plain, lacking logos and the usual publishing and copyright legalities.

Unless someone comes forward with contradictory information, most collectors believe the Russian items are not officially licensed releases.  They may be rare, though, so in one sense the items can be seen as sought after collectibles.  Based on the scans and info provided for this item, I'd agree this is not official.  There is another Russian Nevermind LP with a similar catalog number, OO 004, and an In Utero LP, AA-0004.
